Classification of Early Steps
Providers
To enroll as a provider in Early Steps you must be
able to access the Internet and provide an email address on your
application. The process of provider enrollment involves three easy
steps:
Step One: Complete and
submit by email the appropriate application form. You will
receive an email acknowledgement of receipt of your application
and information you will need to access the required on-line
training modules. Read the specific enrollment requirements for
each class and access to the application.
Step Two: Read and sign the Terms and Conditions of Early
Steps Provider Enrollment and Approval. Click on the form title
to access this form. This form must be signed and either faxed
or mailed to the Early Steps Enrollment Specialist with required
documentation that may include certifications, liability
insurance, diplomas, experience, and fingerprint cards for
background checks (must be mailed).
Step Three: Complete the required Early Steps Orientation
Training on-line training modules.
There are six different provider classes:
- Licensed Healing Arts
- Infant Toddler and Developmental Specialist
(ITDS)
- Service Coordination
- Other Non-Licensed Professional
- Non-Direct Service Staff
- Out-of-Network Provider
- Providers in the ITDS and Other Non-licensed
Professional classes must submit to a criminal
history check, unless you can provide documentation of
clearance on a Level II (FBI) background screening within
the last five years.
- Procedure is to first obtain a finger print card
from the
local Early Steps

then
- send a "Cashiers Check" or "Money Order" for $47.00
payable to the "Florida Department of Health" to the address
found on the terms
and conditions of enrollment

MedicaidSome licensed healing arts
providers are required to apply as an Early Intervention Medicaid
Provider which includes completing the following specials
forms:
Early Intervention Certification

Early Intervention Provider Training

as well as the
Medicaid application . (pdf - 155kb)
Providers who are enrolling as Service Coordinators are also
required
to complete the following special form
Medicaid Case Management Training
form (pdf - 37kb).
