Provider Class: Out of Network Providers

Provider Types

The following providers/entities must complete a short application, which does not require approval, in the Out of Network Provider class of the CMS/ES Provider Management system.

• Local Educational Agency(LEA) Employees
• Florida School for the Deaf and Blind Employees
• Insurance Network Providers
• Equipment Vendors
• Interpreters

The CMS/ES Provider Management System does not approve provider agencies. Completion of Early Steps Online Orientation Modules (1-3): is recommended, but not required for Out-of-Network providers
